Self-Pay and Office Fees
All major credit cards are accepted for payment.
Fees not covered by insurance
Appointments must be confirmed at least 24 hours in advance. Unconfirmed appointments are subject to cancellation.
Late cancellations or no-show fees are automatically applied to the card on file, and missed appointment fees must be paid before a new appointment can be scheduled.
- Initial appointment no-show fee: $200
- Follow-up appointment no-show fee: $100
- Late arrival up to 15 minutes, then no-show fee applies
Other miscellaneous fees
- Medication refills outside of an appointment: $25
- Letters to employers, schools, lawyers, and similar requests: provider hourly rate ($250/hour)
- Comprehensive chart reviews: provider hourly rate ($250/hour)
- Consultation with schools, insurance plans, and similar requests: provider hourly rate ($250/hour)
- Medical records: $1 per page for the first 25 pages, then $0.25 per page thereafter
- Drug tests: $25
